“Future Care Models: Shifting from Places to Populations” brings together diverse voices—from the patient to health system leaders with first-hand experience in home and virtual care. This webinar will offer practical insights into the shift from traditional care settings toward more patient-centred models. Through accreditation, co-creation and evidence-informed approaches, we will explore future models of care delivery.
