This webinar will offer practical insights into the shift from traditional care settings toward more patient-centred models. Through accreditation, co-creation and evidence-informed approaches, we will explore future models of care delivery.

Bold transformation is needed to integrate care across the health ecosystem and communities. Achieving this requires an environment that inspires innovation through co-creation and a shift from traditional care settings toward more patient-centred delivery models. Our panel of presenters will unpack emerging opportunities as we shift from places to population-focused models, integrating home and virtual care into new, more dynamic approaches to quality.
